The US Census Bureau reports that by 2034, the number of older adults will outnumber children for the first time in U.S. history. Not only that, but the population of individuals 65 and older is expected to nearly double over the next 30 years. As the number of older adults rises, the need for care increases.
The AFHS module is designed to equip healthcare professionals with the tools and knowledge to provide our aging population with high-quality, person-centered care. It focuses on the 4Ms framework:
What Matters
Who Knows What Matters Most?
Determinants of Health and Resilience in the Native Hawaiian Community
Medication
Medication Management Through the Lens of the 4Ms
Medication Safety: Managing Pain
Mentation
How Dementia Affects the Senses & Hand-Under-Hand for Positive Caregiving
Recognizing and Addressing Depression in the Nursing Home
Mobility
Falls Assessment and Evaluation
Physical Therapy: Maintaining and Maximizing Function
Completing this on-demand training will give you insights into evidence-based practices that can significantly enhance the quality of life for our kūpuna (elders). The module includes eight videos, each focusing on a specific aspect of the 4Ms, ensuring a comprehensive understanding of age-friendly care.
