Integrating Learning with Real-Time Care

Telehealth training at PI-GWEP integrates clinical learning with real-time care delivery, creating opportunities for trainees to develop skills in clinical decision-making, communication, and care coordination across settings. Through virtual consultations and interdisciplinary team-based models, trainees engage in patient-centered care while working alongside experienced providers, gaining insight into how care is delivered across geographically diverse communities.

Training Within Active Clinical Services

Training is embedded within active clinical services, allowing trainees to participate directly in patient care. Sessions are designed to bring together case-based discussion and real-time clinical application, reflecting how telehealth is used in practice to support complex care needs in older adult populations.

Geriatrics Telehealth Consult Clinic

The geriatrics telehealth consult clinic, conducted in partnership with Queen’s University Medical Group (QUMG), provides trainees with exposure to specialty consultation for older adults. Learners from medicine, nursing, social work, and pharmacy take part in interdisciplinary case discussions and contribute to care planning, experiencing a model that supports both education and service delivery. This approach is particularly impactful for patients in underserved or geographically isolated communities, where access to specialty care may otherwise be limited.

Support for Care Homes on Neighbor Islands

Through the telehealth consult clinic, PI-GWEP extends geriatrics expertise to care homes across neighbor islands, connecting local care teams with an interprofessional team of specialists. This model supports residents with complex needs while providing trainees with exposure to real-world challenges in rural and underserved settings. Telehealth consultations address a range of clinical concerns, including behavioral changes related to dementia, sleep disturbances, medication management, fall risk, and care planning. By working alongside geriatricians, pharmacists, social workers, and nurses, trainees observe how coordinated, team-based care is delivered across distance and resource-limited environments.

Interdisciplinary Team-Based Care (GRACE Model)

Trainees may also participate in interdisciplinary team-based care models such as GRACE TeamCare, conducted in collaboration with AlohaCare, where complex patient cases are reviewed collaboratively. These experiences emphasize coordinated care, shared decision-making, and the integration of medical and social perspectives in managing the needs of older adults.

Telehealth training extends clinical education beyond traditional settings by connecting trainees with patients and providers across islands and regions. This approach strengthens workforce capacity while addressing geographic barriers to care. Across telehealth settings, trainees gain experience working within distributed care teams, building the skills needed to deliver patient-centered care beyond traditional clinical environments.
